WellPath Wellness Program

The SRP-MIC Wellness Program, called WellPath, is a voluntary wellness program available to all employees of SRP-MIC and their dependents. The WellPath Program is designed to promote health and prevent disease.  The program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others.

WellPath is a valuable benefit serving more than 2,260 active employees and 1,000 dependents who are enrolled in the SRP-MIC health plan.  Consistently places among the top wellness programs in Arizona, and recognized nationally, uses employee feedback to remove barriers and provide cutting-edge Fitness, fiscal, and nutritional programming. No matter where you are in your wellness journey, you are welcome to start participating anytime you would like, and as much as you would like. Each year participants choose from over 450 endorsed events at the worksite, in the Community, and online. Employees participate on weekends, evenings, before, during and after the workday. You can even participate locally where you want as programming is convenient and available to you, not only where you work.

If you choose to participate in the WelPath Program, you are encouraged to complete Know Your Numbers (KYN). KYN is a 3-part process which includes an Attestation, a Health Risk Assessment, and a Biometric Screening. The KYN Health Risk Assessment is a questionnaire that asks about your health-related activities, behaviors, and medical conditions (e.g., cancer, diabetes, or heart disease). The KYN Biometric Screening includes measurements for height, weight, blood pressure, and a mini blood panel for cholesterol, glucose, and triglycerides, so you can Know Your Numbers. Although KYN is not a requirement to participate in WellPath or receive incentives, it helps you identify risk factors early, which can ultimately delay or prevent disease completely.

Visit the WellPath web site from any web-enabled device, 24/7 at www.wellpath.info. There you will find more information about participation and earning incentives.
