How to Participate in the SRPMIC WellPath Program

The following SRPMIC entities and affiliates are considered "participating" entities in the WellPath Program:

Each year employees renew their WellPath participation by completing three steps:

- Know Your Numbers (KYN): WellPath participants have a keen understanding of their health status and are active in improving their personal wellness. We are all unaware of certain factors (our "Numbers") which have a strong influence on our health, unless we have them measured. WellPath participants Know Their Numbers by participating in worksite screenings or through visits with their physician. If you are having blood work done with your physician, have your physician sign the KYN Verification Form and submit it to WellPath.
- Personal Wellness Assessment (PWA): Now that you Know Your Numbers, use Your Numbers to complete a confidential Personal Wellness Assessment (PWA) at www.myameriben.com. Use WellPath's step by step instructions and Your Numbers to complete the PWA. Note: WellPath receives no personal health information about WellPath participants.
- Check in with a WellPath Coach: WellPath Coaches help WellPath participants participate in the program including regular Exercise/Fitness Participation (EFPs) and to set Health Enhancement Goals (HEGs). The check-in appointment takes approximately 15 minutes and can be scheduled by email or phone.
When you complete these three steps you are a WellPath Participant and earn your first incentive. Check the incentive matrix for your employer.
