This is a place where participants can find answers to frequently asked questions about the WellPath Program. Participants can email their own questions about WellPath to WELLNESS@srpmic-nsn.gov. Questions will be answered and may be posted in this FAQ.

Employees can email their own questions about WellPath to WELLNESS@srpmic-nsn.gov. Questions will be reviewed and, if of interest to everyone, will be posted, along with the answers.
- Q: How do I become a participant in WellPath?
- A: There are three first steps to becoming a WellPath participant:
- Know Your Numbers (i.e. Complete a biometric screening)
- Complete a confidential Personal Wellness Assessment
- Make a check-in appointment with a WellPath coach
You can still participate in and complete events and challenges, but you will not begin to earn incentives until these first three steps are completed.
- Q: What if I miss the onsite Biometric Screenings? Do I have to wait another whole year to "Know My Numbers?"
- A: No, you can complete a biometric screening any time. Use the Know Your Numbers Screening Verification Form, take it to your regular doctor and have him/her sign it indicating that you completed a screening, and submit the completed form to andrew.weiler@srpmic-nsn.gov.
- Q: How do I schedule time with a WellPath coach?
- A: Use the links on the Reference Materials page of this site, click on "WellPath Coaching Schedule" for available coaching times. Call 362-5555 to set up your appointment, or email WELLNESS@srpmic-nsn.gov.
- Q: Can I use payroll deduction to sign my family members up for a Scottsdale Community College Fitness and Wellness Center membership?
- A: No. Payroll deductions taken from an employee's check can only be for that employee's membership. Family members can sign up for an SCC Fitness & Wellness Center (FWC) membership directly with the FWC. They are not eligible for the discounted corporate rate, however.
- Q: Does WellPath receive personal health information (PHI) about me?
- A: No. The only way WellPath would receive PHI about you is if you personally shared it. A representative of the WellPath program will never ask you for PHI and it will never be a condition of an employee's participation in the WellPath Program. When an employee completes the Personal Wellness Assessment online, WellPath receives no information with which it could identify with a specific employee. WellPath only receives "de-identified," group-level information to help design wellness programming that may meet the needs of our employees who voluntarily participate in the program. When an employee completes a Know Your Numbers screening, WellPath receives no PHI which could be identified with a specific employee. Employees may choose to share PHI with a WellPath coach voluntarily, but the employee would have to provide the PHI.
- Q: How do I gain access to the third floor fitness and shower rooms?
- A: Make an appointment with WellPath to review the policies and procedures (usage guidelines) and sign the liability waiver. At this time a WellPath coach will review the equipment, discuss the WellPath Exercise/Fitness Participation Incentive and answer any questions you may have.
