Basic applicant requirements: (1) Must be enrolled Salt River Community member between the ages of 5 - 17. Proof of enrollment required. (2) Must be enrolled in school. Proof of enrollment required. (3) Must submit a written statement for requested item. (4) Must submit completed application. Application requests must be in line with the annual goals of the SRACCY which are all related to education: Academic Success, Attendance, Extra Curricular Activities and/or Personal Development. Applications can be picked up the Youth Services Department, Tribal Administration, HHS Intake Center, HHS Administration or the Education Administration. Applications are due on the 5th of each month and will be accepted until the end of the grant year, February 27th or until funding is exhausted, whichever comes first. S.R.A.C.C.Y. Award & Grant Changes At the general meeting of the Salt River Advisory Council for Children and Youth in August of 2009, revisions to the award and grant requirements were recommended in order to serve more Community youth. The changes/additions are as follows: Criteria for Awards & Grants: Revised: 4. One award per year (March 1 - February 28), not to exceed $500. One grant per year, not to exceed $2000. If a request is denied, the applicant can continue to submit requests. The subsequent request may not be for the same purpose as the denied request. A. Application Requirements for Awards/Grants: Revised: 1. An individual applicant (student) must be an enrolled Salt River Tribal Member that is between the ages of 5 and 17 by the 5th of the month in which he/she applies for an award. Proof of enrollment will be required by providing original SRPMIC tribal ID or Certification of Indian Blood. A copy will be made. Additional Requirement: 5. Applicant must be enrolled and attending school. Proof of school registration will be required i.e. report card, class schedule, letter from attending school, etc. Home schooling does not qualify per Salt River Code of Ordinances. B. Award Options: Revised: 2. Computer Requests: a. Applicants must be in Jr. High or High School - up to the age of 17. Revised: 4. School Supplies: a. Elementary students (grade K - 6) are eligible to receive a maximum of $40. b. Junior high students (grade 7 & 8) are eligible to receive a maximum of $50. c. High school students (grade 9 - 12) are eligible to receive a maximum of $60. D. Application Process: Revised: 1. A completed award or grant application and required documents must be submitted to the Youth Services Department office by the 5th of the month to be considered for that month. Applications received after the 5th will be reviewed the following month. If the 5th falls on the week-end, applications can be submitted the following Monday. 2. The Grants & Awards Sub-committee meets the 2nd week of the month to review all applications. Parents or Guardians will be notified of application status via phone call and/or letter. Applications are reviewed and approved by a Sub-committee which consists of five Salt River Community members. The Sub-committee meets the second Wednesday of each month. You will be notified of the outcome of your application via phone call or letter within one week of the sub-committee meeting. Applications for Award & Grant Guidelines can be picked up at the Youth Services Administration office, Department of Health & Human Services Intake Center & Administration offices, Education Administration office and Tribal Administration office. Completed applications must be submitted to the Youth Services Administration office by the 5th of each month to be considered for that month.
